Family Register Card (Kartu Keluarga) Update

Based on the attached letter from the Department of Education (Letter from Kepala Pusdatin, Kemdikbudristek no: 1846/J1/DS.00.01/2022)  which states that all student data in Dapodik will be synchronized with the Disdukcapil database, we kindly request parents to upload your updated Family Card (Kartu Keluarga) that includes the QR code on the bottom right corner via the form with following link: https://form.jotform.com/cahayabangsa/family-card-update

By providing the latest Family Card (Kartu Keluarga), you will ensure your child's data can be inputted into the Dapodik system to obtain a diploma at the end of their education and for other educational needs such as continuing education to a higher level, applying for scholarships, or participating in government competitions.

If you have not yet received the updated Family Card (with the QR code), you can update it by following this Lost Card Procedure or by contacting: Disdukcapil Bandung (Jl. Berdasarkan surat terlampir dari Kementrian Pendidikan (Kepala Pusdatin, Kemdikbudristek no: 1846/J1/DS.00.01/2022, tentang Surat Edaran Residu NIK)  yang menyatakan bahwa seluruh data siswa dalam Dapodik akan di sinkronisasi dengan database Disdukcapil, kami menghimbau kepada orang tua untuk mengunggah Kartu Keluarga (KK) terbaru yang telah diperbarui dan mencantumkan kode QR pada bagian kanan bawah melalui tautan berikut: https://form.jotform.com/cahayabangsa/family-card-update

Dengan memberikan Kartu keluarga terbaru, Anda akan memastikan data Anak Anda dapat diinput ke sistem Dapodik untuk mendapat ijazah di akhir jenjang dan keperluan pendidikan lain seperti melanjutkan pendidikan ke jenjang lebih tinggi, mendapatkan beasiswa, atau mengikuti perlombaan dari pemerintah.
